1. Customer Account Information Form (CAIF)
2. Two (2) Specimen Signature Cards

Note: Tax Identification Number (TIN) is required.

Photocopy Requirements:

1. Two (2) valid government-issued IDs
2. Alien Certificate of Registration/Business Certificate/Work Permit (if Resident Foreigner)
3. Authentication of a photocopy of one (1) valid passport (if Non-resident foreign citizen)
4. Photocopy of one (1) valid government issued ID of the parent and Birth Certificate of the minor applicant (if in-trust-for accounts)

Note: (1) ID must not be expired. (2) Photocopy must have both photo and signature. It must be a clear copy. (3) Signature on the ID should match on CAIF. (4) For joint account each party should fill out CAIF individually.

Photocopy of the following Documents duly Certified by the Corporate Secretary

1. SEC Registration
2. Articles of Incorporation
   By-Laws
3. Latest General Information Sheet (GIS) filled with the Securities & Exchange Commissions (SEC)
4. Latest Audited Financial Statements
5. Notarized Board Resolution authorizing corporation to open an account with Sincere Securities Corporation and the designated officers authorized to transact and trade on behalf of the corporation.
6. Photocopy of two (2) identity document issued by the Philippine government of the authorized officers
7. Securities Exchange Commission approval to operate here in the Philippines (for Foreign Company)
